Developmental screenings

Community Consolidated School District 89 conducts developmental screening periodically during the school year for children from the age of 2 years 10 months through 5 years old. The screening includes the use of the Ages & Stages Questionnaire 3rd Ed., Ages & Stages: Social-Emotional 2nd Ed., and play-based assessment of developmental skills. Vision/hearing screenings are provided as well.
Families that want their child to be screened, must complete both of the following screening tools in order to determine needs, prior to a developmental screening appointment:
- The Developmental Screening Information Sheet
- The Ages & Stages Questionnaire, 3rd Ed. (ASQ-3) screening tool (English / Español)
- The Ages & Stages Questionnaire: Social-Emotional, 2nd Ed., (ASQ:SE-2)
These forms take about 30 minutes to complete. The ASQ-3 asks children to perform a series of simple tasks. The tasks are designed to identify whether or not children are meeting certain communication, motor/movement, problem solving, and social milestones for their age. The results of the screening highlight the student’s strengths and areas for growth.
Please complete both the ASQ-3 and ASQ:SE-2 questionnaire and the Information Sheet online. Information will be treated confidentially. By providing your child’s information, it is understood that it can be reviewed by the CCSD 89 early childhood team. Following review and scoring, you will receive notification of results. If additional screening appears appropriate, you will be asked to schedule an appointment.

Registration for preschool
Application and registration for the CCSD 89 tuition-based preschool is a separate process. Parents would still need to complete the registration process.
You are not required to complete a preschool screening to register for preschool.
